How to Request Weapons You must submit a completely filled out form to your State Coordinator (1033 Program, One Foundation Circle, Waunakee, WI 53597-8914) requesting weapons. The State coordinator will then approve your request and send it to LESO for their approval. LESO denies or approves your request and sends a letter stating that decision. About 6 weeks after that letter, LESO sends out a checklist stating the amount that needs to be sent in for shipping costs. Once that check is received and processed, your weapons will be shipping directly to you. Upon receiving the weapons, an ATF Form 10 must be filled out for each weapon. Copies of the ATF 10 need to be sent to your State Cooridnator and to ATF. Below are links to a memo explaining how to request weapons, a sample weapons letter, and ATF 10 form.
