How do I enroll in the 1033 Program? There are five forms that need to be completed and signed to be enrolled in the 1033 Program. If this is the first time that you are enrolling in the 1033 Program, LESO is requiring a mission statement from each new LEA. In that mission statement must be some lanaguage that states that the LEA has arrest and apprehension authority. Read this memo to learn about the enrollment fee and a few options on payment of it. - Memorandum of Agreement (MOA): The MOA is an agreement between the Office of Justice Assistance and the Defense Logistics Agency (DLA).
- Enrollment Form: The enrollment form is a commitment from the LEA to pay the $500 annual enrollment fee for the current year.
- Data Sheet: The information from the Application/Data Sheet will determine the quantity of items your department will be allowed to obtain. The Applicaiton/Data Sheet should be kept current and can be updated when needed. Each year, the Law Enforcement Agency (LEA) will be asked to complete a new Application/Data Sheet.
- State Plan: Your signature on the last page of the State's 1033 Program Plans, Policies and Procedures document simply means that your office will comply with the rules of the 1033 Program.
- Disposal Policy: Each LEA needs to have a disposal policy on file. This is a sample of a disposal policy.
